As a courtesy to our patients at Mountain View Vision Center, we authorize and submit to all participating insurance companies.
It is our goal to help our patients maximize their insurance benefit and get the most for their money.
We currently take most insurance plans, such as:

In order to better serve you we require that we have a copy of your insurance card before submitting for payment.
If you ever have any questions concerning your insurance coverage our staff is happy to answer any questions or call your insurance carrier for you.
Information you'll need to bring with you
On your first visit, you should bring your health insurance card(s), and be prepared to provide any applicable information for the following:
- Name of your medical doctor
- Eyewear history, including what types of glasses or contact lenses you are wearing
- Medications you are taking, including aspirin, over-the-counter medications and remedies, oral contraceptives, prescriptions, etc.
- Social history, including your occupation, and whether you spend time reading, using a computer, ski, golf, fish, swim or bike, etc.
- Family history of medical conditions including, blindness, crossed eyes, color blindness, diabetes, glaucoma, high blood pressure, cancer, kidney disease, macular degeneration, retina disease, retina detachment, thyroid problems, etc.
- Your medial history, including whether you have had problems such as fever, weight loss/gain, pregnancy, nursing, skin problems, headaches, migraines, seizures, thyroid, allergies, post-nasal drip, sinus congestion, chronic cough, asthma, bronchitis, emphysema, diabetes, high blood pressure, heart, vascular disease, kidney, bladder, arthritis, anemia, allergies to various medications, etc.
- Any vision problems you may be experiencing, including
- Loss of vision
- Blurred vision
- Distorted vision/halos
- Loss of side vision
- Double vision
- Dryness
- Mucous Discharge
- Redness
- Sandy or Gritty Feeling
- Itching
- Burning
- Foreign Body Sensation
- Excess tearing/watering
- Glare/light sensitivity
- Eye pain or soreness
- Chronic infection of eye or lid
- Sties or Chalazion
- Flashes/floaters
- Tired eyes
- Crossed eyes
- Lazy eye
- Drooping eyelid
- Prominent eyes
- Glaucoma
- Retinal disease
- Cataracts
- Eye infections
- Eye injury
Please note that our office requires payment at the time of service unless we "accept assignment" for your insurance. Remember that you are responsible if your insurance doesn't pay. Contact lens fit and follow-up care are billed separately. Also, please note that refraction will not be covered by Medicare.
